- Proof of Residency: Current bill (utility/rent, etc) Current food stamps/Medicaid letter
- Proof of Income: Pay stubs OR Current food stamps/Medicaid letter
- Proof of Guardianship: Current Medicaid/Peachcare letter, Birth certificate or Social Security card
